Do you have excellent communication and coordinating skills? Jefferson County's Coordinated Services Team is part of the Children, Youth and Families Division in Human Services and provides intervention services to children, youth and families who reside in Jefferson County.
The team provides services that include assessment and case plans for families, parenting education, monitoring and supervision, and case management.
We are looking to add an experienced and compassionate Community Outreach Worker, who will ensure community safety, placement prevention, competency building, and positive client development by coaching and educating children, youth and their families in the community to develop independent living skills, coping strategies, skills to enhance daily living, and peer relational skills and values and overall wellbeing.
Position Summary To be successful in this role, you will be responsible for these key
Responsibilities:
Mentor and educate clients in developing independent living skills and values.
Works within the care coordinator's established family plans to increase client's independence and achieve success.
Assist care coordinators in assessing clients by providing case planning and reviewing goals.
Co-facilitate the Incredible Years social skills group or other groups deemed appropriate for this population.
Assist in coordinating needed services and providing appropriate referrals to available resources including legal, academic, medical, community outreach programs, and multi- disciplinary county programs.
Maintain contact or provides referrals to collateral contacts such as schools, medical offices, and law enforcement.
Variety of other tasks necessary included on the posted job description Salary Information Starting wage:
